Membership Medicine and You


How does our practice differ from the typical traditional practice?

You will experience the difference immediately when you visit us at Engage Health. Our reception area is relaxing and comfortable, and our office staff is friendly. There is little, if any, waiting time. We will take the time to address your health concerns and questions, and your office visits will never feel rushed. Our membership-based practice is limited to just a few hundred, not thousands, of patients, so we are able to get to know our patients, take a more proactive role in their health and wellness, and be available when they need us.


What is membership medicine?

Membership medicine, sometimes referred to as “Concierge” medicine, is a practice model that prioritizes wellness, disease prevention, and personalized care. By eliminating a dependency on limited insurance reimbursements, we are never confined by 10-minute appointments or daily patient quotas. In exchange for an affordable monthly fee, we are able to focus on what matters most: our patients. We are able to take as much time as we need to provide each of our members with the highest quality, patient-centered care, without compromise, so they can live better, longer. As a result, we are able to build a strong doctor-patient relationship, a key component in helping our members stay healthy.


What does membership include?

Your membership includes an annual comprehensive physical exam that extends beyond insurance-covered or Medicare-covered annual wellness visits. We educate you about lifestyle choices and habits and the role they play in your health. We offer thorough assessments, customized labs, and specialized testing to gain a deeper understanding of your individual health condition, as well as to inform our strategies to optimize your health. In addition, our deeper focus on lifestyle assessments includes initial evaluations with our fitness, nutrition, mental health, and health coaching specialists; additional sessions can be arranged if needed. The result is a customized health and wellness plan that includes individualized follow-up and support throughout the year, as well as a far more connected version of primary care designed to prevent disease and achieve optimum health. You have direct and prompt communication with our team. You are seen by your doctor in person or via telehealth, phone, or text–every visit, every call, every time. Same- or next-day office or virtual visits are always available with your doctor so you can address pressing medical issues quickly and gain peace of mind. Our membership model often leads to savings over time by eliminating unnecessary urgent care visits, ER visits, specialty care, and medications.


Will I still benefit from membership even if I don’t expect to need frequent medical attention?

Absolutely! In fact, we believe patients would see their doctor more if it were easier to communicate, if the care was more individualized, and the process was hassle-free. At Engage Health, we make it easy for our patients to communicate with their doctor and make sure every visit is productive and unhurried. We listen carefully to you to understand how your life, work and family situations may be affecting your health–physically, emotionally, spiritually, and relationally. In addition to providing acute care and illness management, we focus on promoting healthy lifestyle habits in an effort to minimize risk factors and prevent disease. Together, we will create a personalized health and wellness plan to help you live better, longer.


Who does this membership plan work best for?

We provide family medicine for ages 13 and up. People of all ages benefit from the value-added services we offer at Engage Health.

Appointments, Referrals & Care


What happens if I’m admitted to the emergency room or hospital?

We know how stressful and often confusing this experience can be for patients and their families. You will be cared for by the emergency room physician and/or hospitalist team. We will communicate with them, review your plan of care, answer questions, and make recommendations as needed. As your health care advocate, we will communicate directly with you and your loved ones and help you navigate the healthcare system. To ensure recovery, we will provide any necessary follow-up care, including any coordination of specialists.


What if I need specialty care?

As primary care physicians, our comprehensive training allows us to handle most of your healthcare needs. If you need to see a specialist, we will help you find the right doctor. When making referrals, we can consult with your specialist prior to your appointment as needed. After your specialist visit, we will follow up to discuss any of your questions or concerns and, if necessary, arrange additional testing and care management.


What if I need urgent care?

If you have a life-threatening emergency, you should call 911 or go to the ER immediately. Otherwise, you can contact Engage Health when needed. Often, we can diagnose and treat your need without a visit to the ER. In fact, membership medicine has been shown to result in fewer ER and urgent care visits because we can discuss your medical needs when you need to. If appropriate, rest assured we can see you the same or next day!


What if I need medical attention while I’m away from home?

No problem! We will coordinate a virtual visit. If you require in-person evaluation and treatment, we will direct you to appropriate nearby care and coordinate follow-up when you return home. Your membership also allows out-of-town students to easily contact us by phone, text, email, or video. Your student will have their own trusted doctor who knows them well and can coordinate their care with local healthcare providers if needed.


What if my doctor is on vacation?

As a member of Engage Health, you are never alone. Even when out of town, your doctor is available by phone for your urgent needs. You will be notified by email before any vacations. If your doctor is not available, one of our other providers will be available to assist you for any urgent matters.


How Insurance Works in the Practice


Do you bill insurance plans?

Yes, we accept most major insurance plans. Insurance is used in our membership-based practice model just as it would be in a traditional primary care practice. Every insurance plan is different, however, and we are guided by our contractual agreement with each insurance provider.


Can you see me if I have Medicare?

Yes! Both Dr. Johnson and Dr. Hartman are participating providers with Medicare and Medicare Advantage plans. We will bill Medicare on your behalf. Our practice is well suited to care for seniors. Aging can provide challenges that feel overwhelming to the patient, as well as to their caregivers. We have the time and expertise to comprehensively address the multiple, varied, and complex issues that may arise.


Do I still need health insurance?

Yes! Membership-based medical practices do not take the place of general health insurance coverage. However, by providing you with better, more connected primary care, your plan utilization can often be reduced.


How to Join


How much is the membership fee?

Membership fees can be paid annually, quarterly, or monthly. Membership fees vary based on individual age and the number of family members. We offer discounts for couples and families.


Can I give a gift of membership?

Yes! It’s a great way to provide your loved ones with support. peace of mind, and personalized attention for their well-being.


Can joining Engage Health save me money?

Yes! We expect many patients will have fewer out-of-pocket medical costs because our smaller practice and more connected care model enables us to better manage the risks of the majority of your health issues and concerns.

In addition, our membership fee is eligible for HSA, FSA or HRA funds. Please check with your tax advisor.


Is there a limit to how many patients can join Engage Health?

Yes. One of the most important aspects of membership-based medicine is limiting the practice size to be able to devote more time and energy to each patient. Therefore, once we have reached our ideal practice size, we will establish a waiting list.


What if I decide to leave the practice?

You may cancel your membership any time after the first three months with a 30-day notice. If you prepaid your membership fee for the year, you will be reimbursed for the remaining months.


Can we meet before joining?

Yes! If you were not an established patient of Dr. Johnson, Dr. Hartman or Gabby Lee, we ask that we meet either in-person (or virtually) to be sure our practice is a good fit for you. Contact us here.


How do I become a member?

You can begin enrollment online now or contact us. Please note, space is limited, and members are accepted on a first come, first served basis. If necessary, your name will be added to a waiting list.
